I also understand that 3 NO-SHOWS will result in not being able to schedule an appointment for 30 days and that I will provide a 24 hour notice if I need to cancel my appointment.

Volunteer waiver: This releases Jesus Provisions Ministries and Care Center from all liability relating to injuries that may occur during any time with JPM including but not limited to event of any kind. By signing into this platform, I agree to hold JPM entirely free from any liability, including financial responsibility for injuries incurred, regardless of whether injuries are caused by negligence.
I also acknowledge the risks involved in any activities. I swear that I am participating voluntarily, and that all risks have been made clear to me.
Additionally, I do not have any conditions that will increase my likelihood of experiencing injuries while engaging in this activity.
By logging into this site, I forfeit all rights to bring a suit against JPM for any reason. I will also make every effort to obey safety precautions as listed in writing and as explained to me verbally. I will ask for clarification when needed. I also OK the use of photograph and understand that my pictures can be used for social media unless otherwise stated.
